We require all cost-share participants to be a cooperator with our conservation district. Please use the attached form to sign up as a District Cooperator. If you have trouble downloading this form, contact us at 304.758.2512.
If you're unsure if you're a cooperator, please fill out a form, and let the district office know when you turn in your application. We'll verify in our system if you are or aren't and take the necessary steps to process your application.
