Complete this form with the requested information. Put a cup of soil in a sandwich bag, attach the form to that soil sample. Make sure you identify each sample with a name or field number to correspond to your farm map. Provide an email address to receive the results. If you want a paper copy of your results mailed to you, include a self-addressed stamped envelope. If you do not receive your results, contact your county WVU Extension Service office.
